Blood Pressure Self-Monitoring Program

The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program at Boone Memorial Health is a free, four-month program that focuses on teaching participants how to accurately monitor their blood pressure and make lifestyle changes to improve their heart health.

  • Training on how to track your blood pressure at home
  • Two personalized support sessions per month with a trained Healthy Heart Ambassador
  • Monthly Nutrition Education Seminars

If you answer YES to any of these questions, the Healthy Heart Ambassador Blood Pressure Self-Monitoring Program is right for you.

  • Do you have high blood pressure or take medication to control your blood pressure?
  • Do you struggle to keep your blood pressure controlled?
  • Do you worry about the health risks of having high blood pressure/

To participate, you must:

  • Be over 18 years old
  • Have a high blood pressure diagnosis
  • Not have had any cardiac events in the previous year
  • Not have an irregular heart beat (atrial fibrillation or other arrhythmias)
  • Not have or not be at risk for lymphedema (swelling in the limbs)

The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program is a four week program. New sessions start periodically throughout the year.
