Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program offered by Boone Memorial Health is a FREE evidence-based program that  was developed by Stanford University and managed by the Self-Management Resource Center.  The program will empower you to take charge of your health and give you tools to better manage your life and feel better.

Education Topics Include:

  • Action-planning, problem-solving, and decision-making
  • Appropriate exercise for maintaining and improving strength and endurance
  • Appropriate use of medication
  • Healthy eating
  • Better breathing techniques
  • Preventing falls
  • Communicating with friends and family
  • Communicating with oneself
  • Communicating with the healthcare system
  • Working more effectively with healthcare providers

Participants will receive the book entitled Living a Healthy Life with Chronic Conditions.

Anyone with two or more chronic conditions is encouraged to participate in the BMH Chronic Disease Self-Management Program.

Examples of qualifying chronic conditions include:

  • Arthritis
  • Asthma
  • Cancer
  • Cardiovascular Disease
  • COPD/Lung Disease
  • Diabetes
  • Thyroid Disease

There are many other chronic conditions that qualify as well.

The Chronic Disease Self-Management Program at Boone Memorial Health is a six week program. New sessions start periodically throughout the year.

Classes lasting approximately 1 to 1.5 hours are held one day each week during the six-week session period.
