Boone Memorial Health is introducing yet another new way for community members to take charge of their heart health with the Healthy Heart Ambassador (HHA) Blood Pressure Self-Monitoring Program. The program is a free, four-month initiative designed to help participants better understand and manage high blood pressure.

High blood pressure is a leading risk factor for heart disease and stroke, yet many people struggle to keep it under control. This program offers simple, effective tools and personalized guidance to help participants improve their health and reduce risks.

When the Program Runs

The Heart Healthy Ambassador Blood Pressure Self-Monitoring Program runs from August 18 through December 15, 2025, with late enrollment accepted through August 26.

Program participants are asked to attend two ten-minute consultations per month during drop-in office hours and attend a nutrition education seminar once a month.

Drop-in office hours throughout the program will be held every Monday between the hours of 2:30 p.m. and 4:30 p.m.

Participants Will Receive

  • A free blood pressure monitor
  • Training on how to track blood pressure at home
  • Two personalized support sessions per month with a trained Healthy Heart Ambassador
  • Monthly Nutrition Education Seminars to support healthier eating
  • Weekly check-ins from the program facilitator by phone, email, or text

Who Can Participate

To qualify, participants must:

  • Be over 18 years old
  • Have a high blood pressure diagnosis
  • Have had no cardiac events in the past year
  • Not have atrial fibrillation, other irregular heartbeats, or lymphedema
