The Social Work Department of Boone Memorial Hospital assist patients, their families and healthcare providers by providing social work services that meet the physical, emotional and spiritual need of our patients at Boone Memorial Hospital. We work to assure that when a patient leaves our care they will go home to the best environment possible to assure their wellness.

Assisting with the formulation of Advance Medical Directives (Living Wills, Medical Power of Attorneys, Healthcare Surrogacy and decisions relating to care at teh end of life).

Assisting with Organ Donation Decisions

Grief Counseling

Crisis Services

Mental Health Screening and Referral

Substance Abuse Screening and Referral

Arranging for in home medical care and home medical equipment and supplies.

Referral to Social Service Agencies such as the West Virginia Department of Health and Human Resources , Meals on Wheels , Alcoholics Anonymous and others.

Assisting with Nursing Home and Long Term Care Placement.

Assisting with referrals to Private Care Homes, Adult Day Care and other elder services.
